- Primary Source Verification of professional license and/or
certification, as required for the position ran within 30 days of
start updated upon expiration.
- Unit specific to the contractor's scope of practice within one
year prior to start date; Updated Annually
- Copies of exams needed, within year of start with passing score
of 80% or higher. Please check document library to ensure
compliance with testing. Document labeled Franciscan Competencies
Requirement.
- Must be current within a year prior to start with passing score
of 80% or higher. Updated Annually. HIPAA, Patient Bill of Rights,
Safety Orientation, Blood borne Pathogens, Fire Safety, Infection
Control, Elder Abuse and Neglect, Age Specific Care, Cultural
Diversity, Advance Directives; renewed annually Please check
document library to ensure compliance with testing. Document
labeled Franciscan Competencies Requirement.
- Proof of 2 immunizations, positive titer or Declination
accepted. If low titer, 1 Booster or declination accepted.
Declination must include a written statement from the candidate
with the candidate's physical signature.
- Must be one of the 3 mask fit types. N95- 3M 1860, 1860s or
1870+. Must be completed within 1 year prior to start. Updated
annually.
- Proof of 3 immunizations, positive titer or Declination
accepted. If low titer, 1 Booster or declination accepted.
Declinations must include a signature and approval on behalf of the
Agency, as well as a signature from the candidate.
- Flu Season dates to be communicated each year. Only
Medical/Religious exemptions will be accepted with a physician's
signature(Medical) or written signed statement from
traveler(Religious). Medical exemption request must be completed on
the FMOLHS Medical Exemption Flu Form, please see Document Library.
All Exemptions must include a signature and approval on behalf of
the Agency, as well as a signature from the candidate.
- Proof of negative Blood test or Skin test as described below. -
QuantiFERON Blood Test within one year prior to start OR 2-Step TB
Skin Test 1 within 30 days and the other within 1 year. If positive
TB record; Must provide MD note stating traveler is free of active
TB or provide proof of treatment for LTBI within 30 days of
start.
- Statement signed and dated within 1 year of start stating
traveler is able to work without restrictions.
- Proof of 2 immunizations, positive titer or Declination
accepted. If low titer, 1 Booster or declination accepted.
Declination must include a written statement from the candidate
with the candidate's physical signature.
- Proof of vaccination within past 10 years or Declination
accepted. Declinations must include a signature and approval on
behalf of the Agency, as well as a signature from the
candidate.
- See document library. Required within 6 months of start. If
POSITIVE documentation of evaluation from the Department of Health
required. If treatment was prescribed, proof of completion
required.
- Resume or Profile
- 2 references required; 1 Charge or Higher; 1 must be supervisor
reference.
- Must be completed within 30 days prior to start. Must include
SSN Address Trace report. All counties reported within 7 years must
be run. County Criminal Searches for all counties lived/worked
in/in school within the last 7 years. State checks will NOT be
accepted for county checks. National Criminal check required. All
background checks must include the NSO, OFAC, OIG, and SAM. May be
listed on background check individually or may be included as part
of the FACIS 3 search- but must list each search was included as
part of the breakdown. Education Verification must be 3rd party.
Employment Verification- the last 5 years of employment required.
Employers must be verified. Driving Record- can be included in the
background check or ran separately. ALL REPORTS MUST BE UPLOADED
TOGETHER
- If not uploaded with background check. To be complete within 6
months of start updated annually.

(Quaaludes).
